Remember: the more complete the information you give, the faster you are approved.
- Prior health insurance information: include the start & end dates, as well as company name & identification number if known.
- Health History: for all “yes” answers make sure to follow up with as detailed as possible an explanation of dates, treatment, & current status (still receiving treatment or is it resolved?)
