1. When completing doctor's information include name, address and phone number of doctor's office so medical records can be obtained quickly.

2. Be specific about medications listed by including prescription name and dosage.

3. Accurately state the start and end dates of illnesses when requested.

4. Make all checks payable to the insurance company not the agent's office.

5. Note your social security number on all checks and money orders.

6. Reference all yes questions on health statements in the appropriate sections on application giving all necessary details.

7. When setting up a pre-authorized checking account deduction plan include a voided check even if paying by credit card or money order.

8. Make a copy of the application for your records. The insurance carriers may have questions during the application process.

9. Include a phone number where you can be reached during the day so if problems or questions arise during underwriting someone can reach you.

10. Always remember to sign and put the date in all indicated places or the application will be returned to you. Signatures often forgotten are those of the spouse and children over eighteen who are listed on the application.


Applications that include all the information stated above are generally processed in half the time of applications, which are missing information. Remember the more complete the information you give is the faster you will be approved.
