KEY ITEMS TO REMEMBER WHEN ENROLLING NEW EMPLOYEES

1) Provide new employees with enrollment applications for ALL lines of coverage.

2) All employees must satisfy the company's probationary period before they can be enrolled in the plan.

3) All enrollment applications require a date of hire and date of enrollment.
Please note: These should be within the company's probationary period recorded with that insurer.

4) If the employee has dependents (spouse or children) he or she should either:
     a) Include them on the application, or
     b) Sign a declination for those dependents

5) Please note when employees decline their dependents the following may apply:

  • A carrier may not have an open enrollment to add employees or dependents at a later date.
  • A late enrollee might be subject to a 12 month waiting period and an additional waiting period for pre-existing condition(s) may apply depending on the carrier.

6) An employee may only add a domestic partner if it is an option in the contract between the employer and the carrier.

7) HMO enrollees must choose a Primary Care Physician. HMO participants MUST obtain a referral from their Primary Care Physician prior to receiving services from another health provider.
